Clinical Solutions will review charts for process breakdown, will repair or redevelop to meet the facilities needs and meet regulatory compliance. We recommend that a consultant visit the facility at least monthly to conduct a random chart review to identify any new areas of concern. By doing so, the facility is able to QA&A the issue and put in place an acceptable plan to prevent further breakdown of the system.
